I think most of the questions are pretty good, I think the one geared towards our project being in the middle and not just a tag on at the end is a good idea. I would put it to Andy and Dan to think about the family question (I know we discussed it a bit in the meeting), because in this example (although only being vague itself) seems to miss something out about the family...
Could have something about the relative that lives closest outside of the house (not closest living relative as I initially put down)?
These look good! My only qualm is with question 5 - some of the options are not mutually exclusive, but the user is only able to select one option even if multiple choices apply to them. E.g.
* Spouse/Partner (Skip Question 6)
* 1+ Siblings (with or without children/grandchildren)
* Children options . . .
The user could theoretically be able to select all three. Perhaps make it a checkbox question instead?
Oooh, additional suggestions on second read through:
-Maybe alter question 10 to read "Do you worry that if you were to have an accident AT HOME, you would be unable to get help?"
-I know we have a nice round number of 10 questions, but is it worth adding in an open question asking for suggestions/user concerns that might be relevant to the project? I'm not sure about this, really, the questions do give good coverage as is, but I wonder whether we might potentially miss out on some good, specific user-originating ideas by excluding open questions entirely.
I should probably note here that this was just the start of a questionnaire, I didn't intend this to be full or complete, I thought it would get us started on what we needed to think about!
Ok guys I have noted all of your feedback on Anna's initial design and I will come up with a revised version of the questionnaire as soon as I can.
